Baby Dedication Application

In an effort to comply with the request of the Registrar General’s Department, re: Registration of Births; it is kindly requested that a copy of the official Registration Certificate be supplied with this application  for the baby dedication.

Your compliance with the above will greatly facilitate your request.

(Please note that baby dedications are conducted every 2nd Sundays of each month; however this may be subject to change at the discretion of the pastor)
